We now offer a facility for booking routine GP appointments, ordering your repeat prescriptions and viewing parts of your medical record via an online web service called Patient Online Access. Please ask the receptionist for your Patient Access Registration information and relevant Registration form so you can apply for your own secure account. Unfortunately it is not possible to apply online for access so you will need to come into the practice to complete the process.

All applicants (patients, parent/guardian of children under 11 years, and those wishing to have proxy access to another person's records) may be asked to provide proof of identity to Cogges Surgery in person. This proof must include some recognised photographic identification e.g. passport, driving licence, plus proof of current address e.g. bank statement.
Patients aged 16 years or over who wish to have access for themselves should complete this form: PatientOnline-REGISTRATION FORM over 16s.pdf
For patients who are aged 16 years or over where someone else wishes to have access on the patient's behalf (as a proxy) this form is to be used Over 16 Proxy Access Registration form . If a patient does not have capacity to consent to this, a proxy/carer can apply on their behalf.
For patients aged 11 to 15 years, there is a separate form that can be completed by the patient and their parent/guardian: Proxy Access Registration Form for 11-15s
For patients aged under 11 years, the parent/guardian should complete Proxy Access Registration Form for 10 and Under
Please note that only access for online appointment booking and repeat medication ordering will be authorised for children under 16 years (unless additional access to the medical record is authorised by their usual doctor in exceptional circumstances)
